At Emerald Tree, we can assist with specialist psychosocial Functional Capacity Assessments, NDIS reports and practical reporting pathways for participants, families, support coordinators, plan nominees, workplaces, schools and referring professionals.

Flexible assessment formats

Our assessments can be completed in person, via telehealth, or, where appropriate, through a home, workplace or school visit. Telehealth is especially helpful if you do not live locally or cannot easily attend the clinic.

A home, workplace or school visit can be useful where real-world context is important. It may help the clinician better understand day-to-day functioning, support needs, routines, participation, communication, environmental barriers and practical challenges.

Clinical suitability note

Home, workplace and school visits are arranged based on clinical suitability, consent, location, availability and the purpose of the report. Additional travel time or costs may apply.

Referral information

Assessment and report enquiries are welcome from participants, families, support coordinators, recovery coaches, allied health providers, GPs and organisations.

Functional Capacity Assessments

Specialist psychosocial Functional Capacity Assessments designed to understand functioning, identify support needs and build a stronger case for the NDIS.

  • Explores daily living, participation, communication, emotional regulation, safety, routines and support needs.
  • Useful for NDIS planning, plan reviews, support coordination and practical support recommendations.
  • Particularly relevant for psychosocial disability including depression, anxiety, PTSD, OCD, trauma and related functional impacts.

NDIS Plan Review Reports

Information to support upcoming plan reviews and funding conversations.

  • Focuses on progress against current goals and what supports may be needed next.
  • Can include functional observations and practical examples for review meetings.

Change of Circumstance Reports

Reports for material changes in support needs, capacity or daily functioning.

  • Documents meaningful changes that may affect care, safety, capacity or support levels.
  • Helps explain why a plan, service or support approach may need to change.

Victims of Crime / VAS Reports

Support reports for eligible clients navigating recovery and service pathways.

  • Supports eligible clients with clinically informed information for recovery pathways.
  • The report scope depends on referral context, consent and the purpose of the request.
